Endodontics (Root Canal Treatment)
Endodontics (Root Canal Treatment)
Endodontics is the part of dentistry that deals with the inner portion of your tooth which is commonly called the nerve. By far the most common endodontic procedure is aroot canal treatment (RCT) or root filling. This procedure involves your dentist removing the inflamed, infected or necrotic nerve from inside your tooth, carefully cleansing the tooth and placing a root filling material into the root where the nerve originally was so that the root is sealed from your mouth and no infection can enter your jaw through the tooth.

Endodontic procedures have evolved a great deal in the last few years and it is now possible to predictably treat and save a tooth that years ago would have required extraction.

Normal tooth anatomy To understand endodontic treatment, it helps to know something about the anatomy of the tooth. Inside the tooth, under the white enamel and a hard layer called the dentin, is a soft tissue called the pulp. The pulp contains blood vessels, nerves, and connective tissue and creates the surrounding hard tissues of the tooth during development.

Infected / Inflamed Tooth Endodontic treatment (root canal) is necessary when the pulp, the soft tissue inside the root canal, becomes inflamed or infected. The inflammation or infection can have a variety of causes: deep decay, repeated dental procedures on the tooth, or a crack or chip in the tooth. In addition, an injury to a tooth may cause pulp damage even if the tooth has no visible chips or cracks. If pulp inflammation or infection is left untreated, it can cause pain or lead to an abscess.

Endodontics - Procedure
Endodontic treatment can often be performed in one or two visits and involves the following steps:
The endodontist examines and x-rays the tooth, then administers local anesthetic. After the tooth is numb, the endodontist places a small protective sheet called a 'dental dam'over the area to isolate the tooth and keep it clean and free of saliva during the procedure.

Access Opening The endodontist makes an opening in the crown of the tooth. Very small instruments are used to clean the pulp from the pulp chamber and root canals and to shape the space for filling.

Bio Mechanical Preparation and Root Filling After the space is cleaned and shaped, the endodontist fills the root canals with a biocompatible material, usually a rubber-like material called 'gutta-percha.'The gutta-percha is placed with an adhesive cement to ensure complete sealing of the root canals. In most cases, a temporary filling is placed to close the opening. The temporary filling will be removed by your dentist before the tooth is restored.

Crown Restoration After the final visit with your endodontist, you must return to your dentist to have a crown or other restoration placed on the tooth to protect and restore it to full function.

To make Endodontics more precise and easy, we, in our clinic have updated ourselves with Rotary Instruments (X-Smart), Apex Locators and Oburators. We also specialise in single sitting root canal treatment.

SINGLE SITTING ROOT CANAL TREATMENT : This treatment is done to avoid multiple injections & multiple visits, thus reducing the patient apprehension.

  • Absolutely painless and extremely comfortable.
  • Generally completed in a single sitting of half an hour to one hour.
  • Performed under total isolation of the tooth using the rubber dam technique.
MICROSCOPIC ENDODONTICS
The endodontic operating microscope is an invaluable device in root canal treatment. Visualization inside the tooth is enhanced with increased magnification and illumination, allowing the dentist to operate very precisely in an area the size of a square millimetre.

During root canal therapy, magnification and illumination provided by the operating microscope aids with access cavity, removal of pulp chamber calcifications, treatment of calcified canal , locating of root canal orifices, identification of cracks and fracture lines ,and the treatment of internal/ external resorptions or perforation repairs .

In endodontic retreatments, the microscope is helpful in identifying and removing left materials, such as sealer remnants, pastes or gutta percha , silver points and carrier-based materials, posts or fractured instruments. In terms of root canal treatment, this precision allows to:

  • Localize and treat the main and secondary root canals of the tooth, including branches often not visible in a standard treatment.
  • Localize and remove broken endodontic instruments, restorative crown fragments, calcified canals.
  • Treating very narrow or obliterated (closed) root canal.
  • Successfully treating difficult and complicated canals with rare root anatomy.
  • Root resorption treatment.
  • Microsurgical apicoectomy and root-end filling with special materials.
